Motion Control allows certified prosthetists to rent Motion Control components when a trial-fit can
help to confirm the best prescription for an individual patient. We encourage trial fittings for challenging
cases, or when component options need to be explored before the optimal prescription can
be made.
- Rental Fee: 10% of list price, invoiced monthly. Terms on invoice are Net 30. All rental invoices
must be paid in full. Customer should have an account established with a credit limit equal to or
greater than the full purchase price of the components.
- The rental period begins upon receipt of the equipment from Motion Control. The rental
period ends upon either purchase of the equipment, or receipt of the equipment at Motion
Control. All rental invoices need to be paid within 30 days of the invoice date regardless of
whether the equipment is returned or purchased. If the components are purchased, a rental
conversion invoice will be generated by Motion Control with rent prorated to the date of
conversion to purchase. The rental conversion invoice and all monthly rental invoices must be
paid within 30 days of the invoice date.
- Percentage of Rent applied at time of purchase:
1st month: 100%
2nd month: 80%
3rd & succeeding months: 60%
6th month: Equipment must be purchased or returned.
- If the equipment is returned to Motion Control, all rental invoices must be paid; no rental
conversion invoice will be generated.
- When the fitting center wishes to purchase the rental equipment, Motion Control must be
notified and given a purchase order. The normal warranty period for purchased equipment will
begin at that point. The applicable rental fees will be credited toward the purchase; and the
balance of the purchase price will be invoiced. The terms of the rental conversion invoice is 30
days.
- Renters of a Utah Arm must be certified as having completed a Utah Arm Fitting Procedures
Training Course. The fitting centers renting the equipment will be responsible for the care of
the equipment and any damage incurred that is not covered by our warranty. Manufacturer’s
warranty will cover the equipment for defects in materials and manufacturing workmanship.
- Rental equipment provided by Motion Control will be new or saleable as new.
Guidelines for Prosthetists Fitting Rental Equipment:
We suggest that the prosthetist completes preliminary fitting steps as much as possible before the
equipment is rented, e.g., check socket fitting, muscle training, etc. If electrodes, alignment brackets,
or other accessories are desired before the rental equipment is delivered, these can be forwarded in
advance.
We strongly suggest that written funding approval be confirmed before the rental period begins.
A commitment should be obtained from the funding agency to purchase the completed prosthesis
once the temporary fitting has demonstrated success at an agreed upon point in time. It is the
responsibility of the prosthetist to verify that the cost of the rental equipment and prosthetic services
will be covered.
Prosthetists renting the equipment will be responsible for any damage to the equipment outside of
normal usage covered in the Motion Control warranty. We advise the prosthetists to arrange an allrisk
rider on the user’s homeowner’s (or renter’s) insurance policy.
